Here's the format that a Directory update request should take:

Full Name of School:  STATE UNIVERSITY OF SOMEPLACE AT SOMEWHERE 
Office Name:           The name of your office (e.g., Counseling Services)
Contact Person:      The name and email address of the person responsible for the web page.
Contents:   Typical examples:
                        Basic information (e.g., services offered)
                        Self-help information
                        Training information (e.g., internship brochure)
                        Links to other internet resources
URL:                 http://some.address.edu/xyz.html
